Key Responsibilities:
- Conduct comprehensive psychosocial assessments to fully understand the needs of hospice patients and their families.
- Develop and implement personalized care plans in collaboration with a multidisciplinary team to optimize patient and family support.
- Provide skilled counseling and emotional support focused on grief, loss, and adapting to end-of-life adjustments.
- Facilitate advance care planning, advising on advance directives and clarifying goals of care for patients and families.
- Connect patients and families with community resources and support systems to ensure comprehensive care.
- Advocate for patient needs, ensuring care is delivered with cultural competence and sensitivity.
- Lead family meetings to support decision-making and provide crisis intervention when required.
- Maintain precise documentation in compliance with regulatory and organizational standards.
- Actively participate in interdisciplinary team meetings and case reviews to coordinate patient care effectively.
- Provide ongoing bereavement support and follow-up services to families after a patient's passing.
Requirements:
- Master’s degree in Social Work (MSW) from an accredited institution.
- Current licensure as an LMSW or LCSW in New Mexico.
- A minimum of 1–2 years of clinical social work experience, preferably within hospice, palliative care, or a healthcare setting.

- Excellent communication, interpersonal, and organizational skills, with the ability to work independently and collaboratively within a team.
- A valid driver’s license and reliable transportation are required for roles involving home visits.
