Position: Assistant Director of Nursing (ADON)/ MDS Coordinator

Edenbrook Rochester West, a skilled nursing facility located in Rochester MN, is searching for an Assistant Director of Nursing (ADON)/ MDS Coordinator.

The Assistant Director of Nursing (ADON) / MDS Coordinator plays a critical role in supporting the day‑to‑day operations of the nursing department to ensure high‑quality resident care in compliance with federal, state, and local regulations. This position provides ongoing leadership support through staff development, clinical oversight, regulatory compliance, and interdisciplinary collaboration.

MDS experience is a plus, but not required. We’re happy to train the right individual who’s eager to grow and advance their career.

The role includes on‑call responsibilities and may require occasional floor coverage as needed.

Responsibilities:

  • Assist in developing and implementing nursing policies, procedures, and regulatory compliance initiatives.
  • Support staff development, training, onboarding, and performance improvement.
  • Oversee infection control practices and wound care management.
  • Conduct routine nursing rounds to monitor care quality and support staff.
  • Ensure resident rights, safety, and protection from abuse or neglect.
  • Participate in interdisciplinary team meetings and assist with operational planning.
  • Collaborate with the DON to mentor nursing staff and maintain a positive team culture.
  • MDS scheduling, set up and completion
  • New admission authorizations, updates, communication with family/insurance providers, communication with case managers

Education and/or Experience:

  • Active RN license in the state of Minnesota
  • Minimum of two years of experience in Long-Term Care
  • Previous nursing supervisory or leadership experience required
  • Prior MDS experience is preferred, though the right candidate will be trained.
  • Wound management experience is a plus.
