Job Title: Admissions Coordinator / Community Liaison

Position Summary: 

The Admissions Coordinator / Community Liaison is responsible for driving census growth through relationship development, referral management, and seamless admission coordination. This role serves as the primary point of contact for potential residents, families, and community partners, ensuring a smooth transition into our skilled nursing facility.

This individual combine sales acumen and exceptional customer service to convert referrals into admissions while maintaining compliance and resident-centered care standards.

Key Responsibilities: 

Admissions & Referral Management: 

  • Takes an active role in morning stand-up meeting communicating bed offers and scheduled admissions.
  • Ensure completion of admission paperwork.
  • Track tour outcomes and conversion ratios.
  • Evaluates facility physical environment through daily assigned rounds and discusses areas of opportunity at morning stand-up meeting.
  • Process owner of the room readiness standard.
  • Participates in discharge follow-up calls / processes.

Business Development & Community Outreach: 

  • Develop and maintain strong relationships with community organizations.
  • Conduct routine in-person visits to assisted living communities, senior centers, senior high-rise apartments and physician offices.
  • Represent the facility at networking events and community programs.
  • Provide education on facility services, specialty programs, and outcomes.
  • Maintain a consistent presence in assigned territories.

Census Growth & Strategy: 

  • Meet or exceed established admission and census growth targets in conjunction with hospital liaison and centralized admissions coordinator.
  • Monitor daily census and proactively identify opportunities for growth.
  • Analyze referral and discharge trends and adjust outreach strategies accordingly.
  • Participate in marketing planning and implementation.

Customer Service & Communication: 

  • Serve as liaison between residents/families and facility leadership during admission process.
  • Conduct tours and provide personalized consultations for prospective residents.
  • Ensure a positive first impression experience.
  • Address concerns promptly and professionally.
  • Maintain confidentiality in accordance with HIPAA regulations.

Qualifications: 

  • Minimum 2 years of experience in healthcare admissions, marketing, business development, or case management (skilled nursing preferred).
  • Strong understanding of Medicare, Medicaid, and managed care reimbursement.
  • Excellent communication, negotiation, and relationship-building skills.
  • Proven ability to meet performance goals and drive results.
  • Valid driver’s license and reliable transportation for community visits.
