Position: Lead Physical Therapist

We are seeking a highly motivated and experienced Lead Physical Therapist to join our dynamic rehabilitation team. In this role, you will oversee the physical therapy department, provide hands-on patient care, and lead our team of therapists and assistants to deliver exceptional outcomes. This position is ideal for someone passionate about both patient wellness and professional mentorship.

Responsibilities:

  • Evaluate patients and develop individualized treatment plans to restore mobility, reduce pain, and improve physical function.
  • Lead and supervise the physical therapy team, ensuring consistent delivery of high-quality care.
  • Collaborate with interdisciplinary team members, including occupational and speech therapists, nurses, and physicians, to provide a holistic approach to care.
  • Monitor patient progress and modify treatment plans as needed.
  • Maintain accurate documentation and ensure compliance with all regulatory guidelines.
  • Conduct staff training, performance reviews, and mentorship for team development.
  • Champion best practices, evidence-based care, and ongoing improvement initiatives.
  • Support discharge planning and provide patient/family education on home care strategies.

Qualifications:

  • Licensed Physical Therapist (PT) in the state of Minnesota (or eligibility to obtain licensure).
  • Minimum of 3 years of clinical experience in a skilled nursing or rehabilitation setting.
  • Previous leadership or supervisory experience preferred.
  • Strong clinical knowledge, leadership ability, and organizational skills.
  • Excellent communication, teamwork, and interpersonal skills.
  • Passion for serving seniors and enhancing their quality of life.
