Oasis Therapy Services is seeking an Occupational Therapist to provide one-on-one rehabilitation services at Eden Vista Litchfield, an independent living, assisted living, and memory care community in Litchfield, Minnesota.

Responsibilities

  • Evaluate residents and develop individualized occupational therapy plans.
  • Provide one-on-one treatment focused on activities of daily living, functional mobility, safety, and independence.
  • Monitor residents’ progress and modify treatment plans as needed.
  • Complete discharge planning and provide recommendations for continued care.
  • Educate residents and caregivers on exercises, adaptive techniques, and equipment.
  • Direct and supervise Occupational Therapy Assistants as appropriate.
  • Complete accurate Net Health documentation and collaborate with the care team.

Qualifications

  • Active and current Minnesota Occupational Therapist license.
  • Active NBCOT OTR certification in good standing.
  • Graduate of an ACOTE-accredited Occupational Therapy program.
  • Current CPR certification.
  • Strong communication, organization, and documentation skills.
  • Geriatric, independent living, assisted living, memory care, skilled nursing, or rehabilitation experience is preferred.
