Oasis Therapy Services is seeking a Team Lead – Occupational Therapist (OT) to provide one-on-one rehabilitation services, participate in care conferences, serve as a liaison with residents and staff, and assist the Director of Rehabilitation (DOR) with some managerial tasks in an assisted living setting in Cannon Falls, Minnesota.

Responsibilities

  • Direct the resident evaluation and reevaluation process.
  • Develop and document individualized occupational therapy intervention plans.
  • Provide therapy focused on ADLs, mobility, safety, and independence.
  • Monitor residents’ progress and modify intervention plans as needed.
  • Direct and document discharge planning and recommendations.
  • Participate in resident care conferences and coordinate occupational therapy services with the interdisciplinary care team.
  • Serve as a liaison among residents, facility staff, and the therapy department.
  • Assist the DOR with some administrative, coordination, and managerial tasks.
  • Provide and document required clinical supervision of Occupational Therapy Assistants when assigned.
  • Educate residents and caregivers on exercises, adaptive techniques, and equipment.
  • Complete accurate Net Health documentation and collaborate with the care team.

Qualifications

  • Active and current Minnesota Occupational Therapy license as an Occupational Therapist.
  • Graduate of an accredited Occupational Therapy program.
  • Current CPR certification.
  • Strong leadership, communication, organization, and documentation skills.
  • Ability to work independently and collaborate effectively with residents, staff, and therapy professionals.
  • Geriatric, assisted living, skilled nursing, rehabilitation, care conference, or team leadership experience is preferred.
