Oasis Therapy Services is seeking a Team Lead – Certified Occupational Therapy Assistant (COTA) to provide one-on-one rehabilitation services, participate in care conferences, serve as a liaison with residents and staff, and assist the Director of Rehabilitation (DOR) with some managerial tasks in an assisted living setting in Cannon Falls, Minnesota.

Responsibilities

  • Provide occupational therapy interventions delegated by and under the supervision of a licensed Occupational Therapist.
  • Carry out individualized intervention plans developed by the supervising Occupational Therapist, focusing on ADLs, mobility, safety, and independence.
  • Participate in resident care conferences by sharing observations and progress updates within the scope of the role.
  • Serve as a liaison among residents, facility staff, and the therapy team.
  • Assist the DOR with some administrative, coordination, scheduling, and managerial tasks.
  • Monitor residents’ progress and report changes, concerns, or the need for reevaluation to the supervising Occupational Therapist.
  • Complete accurate Net Health documentation and collaborate with the care team.

Qualifications

  • Active and current Minnesota Occupational Therapy Assistant license.
  • Active NBCOT COTA certification in good standing.
  • Graduate of an accredited Occupational Therapy Assistant program.
  • Current CPR certification.
  • Strong leadership, communication, organization, and documentation skills.
  • Ability to work independently while following required Occupational Therapist supervision.
  • Geriatric, assisted living, skilled nursing, rehabilitation, care conference, or team leadership experience is preferred.
