Edenbrook Second Ave, part of the Eden Senior Care Network, a skilled nursing facility is seeking for a Transportation Driver for our wonderful skilled nursing facility!

Position: Transportation Driver / CNA (Full-Time)

The primary responsibility of the Transportation Driver / CNA (Full-Time) is to safely transport residents to and from medical appointments, therapy visits, and other scheduled destinations while ensuring their comfort and safety. In addition to transportation duties, this role requires an active Certified Nursing Assistant (CNA) who is willing and able to provide direct resident care and assist the nursing team with CNA responsibilities as needed to support daily operations and deliver high-quality care.

Schedule: Monday–Friday, typically 8:00 AM – 4:30 PM. Must be flexible to accommodate resident appointment schedules.

Job Responsibilities:

  • Transport residents safely to and from medical appointments and other approved destinations.
  • Review daily and weekly transportation schedules with the Transportation Coordinator.
  • Prepare residents for appointments, ensuring proper hygiene, attire, and comfort.
  • Gather and maintain all required paperwork and documentation before transport.
  • Assist residents with entering and exiting the transportation vehicle, including the use of wheelchair lifts and other mobility equipment.
  • Accompany residents to appointments as needed and provide support throughout the visit.
  • Communicate information, instructions, and updates received from healthcare providers to the nursing team.
  • Monitor resident safety and well-being during transport and appointments.
  • Report transportation concerns, scheduling conflicts, or resident issues to the appropriate staff.
  • Maintain resident confidentiality and uphold resident rights at all times.
  • Provide CNA-related support and resident care as needed within the facility.

Qualifications:

  • Current Certified Nursing Assistant (CNA) certification required.
  • Valid Pennsylvania driver's license required.
  • Ability to read, understand, and follow written and verbal instructions.
  • Strong communication and interpersonal skills.
  • Ability to work professionally with residents, families, staff, healthcare providers, and the general public.
  • Clean driving record with fewer than 15 points on Motor Vehicle Report (MVR).
  • Ability to safely lift, transfer, and assist residents, including the use of wheelchair lifts and mobility equipment.
  • Ability to receive, understand, and relay detailed medical information to nursing staff accurately.
