Oasis Therapy Services is seeking a Physical Therapist to provide individualized, one-on-one rehabilitation services to seniors in independent living, assisted living, and memory care settings in Apple Valley, Minnesota

 Responsibilities

  • Evaluate residents and develop individualized physical therapy treatment plans.
  • Provide treatment focused on strength, balance, gait, mobility, transfers, and fall prevention.
  • Monitor residents’ progress and modify treatment plans as needed.
  • Complete discharge planning and recommend continued care.
  • Educate residents, families, and caregivers on exercises, safety, and assistive devices.
  • Supervise Physical Therapist Assistants (PTAs), when assigned, in accordance with Minnesota regulations.
  • Maintain accurate Net Health documentation and collaborate with the care team.

Qualifications

  • Active, current Minnesota Physical Therapist license in good standing.
  • Graduate of a CAPTE-accredited Physical Therapy program or an educational program accepted by the Minnesota Board of Physical Therapy.
  • Current CPR certification.
  • Strong clinical assessment, communication, organization, and documentation skills.
  • Geriatric, senior living, or rehabilitation experience preferred.
  • Net Health EMR experience preferred.
