Position: MDS Coordinator

Williamsport South Rehabilitation & Nursing Center is seeking an experienced and dedicated MDS Coordinator to join our team.

Compensation: Salary is negotiable and based on experience and skill set.

We are looking for a candidate with prior MDS experience and eligibility as an RNAC or LPNAC who is well-versed in MDS, RAI, and Clinical Reimbursement. This is a leadership role, and the ideal candidate should be comfortable guiding the team in the duties and responsibilities listed below. Prior experience with PointClickCare (PCC) is preferred. **Willing to train the right candidate**

Essential Duties and Responsibilities:

  • Coordinate MDS scheduling, setup, and completion.
  • Manage new admission authorizations, updates, and communication with families, insurance providers, and case managers.
  • Perform ICD-10 coding for new admissions and provide ongoing updates for the LTC population.
  • Participate in implementing, updating, and reviewing resident care plans.
  • Communicate with families and residents regarding care plans, insurance needs, and discharge needs; attend care conferences.
  • Educate the IDT on MDS, assessments, and skilled care service requirements.
  • Lead Utilization Review Meetings and ensure PDPM scores and rates are accurate.
  • Review and train IDT/staff to ensure proper documentation is maintained.
  • Participate in facility Quality Measures and QAPI processes.
  • Conduct chart reviews and audits as needed.
  • Assist the IDT during the survey process.

Qualifications

  • Active license as a Registered Nurse (RN) or Licensed Practical Nurse (LPN) with eligibility to practice in Pennsylvania.
  • Strong knowledge of the MDS process with 3+ years of experience preferred.
  • In-depth knowledge of ICD-10 coding and guidelines, Medicare Chapter 8 Manual, and RAI Manual.
  • Exceptional organizational skills; proficiency in Excel, Teams, or Zoom platforms is a plus.
  • Excellent written and oral communication skills.
